Report a Form Security Issue to Squarespace
Collect custom information from customers during checkout by adding a personalized form. Here's how to create and manage a custom checkout form:
Enable Custom Checkout Form:
- Navigate to the Checkout panel
- Scroll to Additional Fields
- Check "Enable Custom Checkout Form"
- Click "Edit Custom Checkout Form"
Create Your Form:
- Enter a form name (e.g., "Gift Message" or "Delivery Instructions")
- Click "Add Form Field" to create new fields
- For each field:
- Add a title and description
- Mark as required if necessary
- Click "Done" to save the field

Customize Field Options:
- Edit field content anytime
- Remove fields using the trash can icon
- Ensure at least one field exists
- Check "Required" for mandatory fields
