Report a Form Security Issue to Squarespace

Collect custom information from customers during checkout by adding a personalized form. Here's how to create and manage a custom checkout form:

Enable Custom Checkout Form:

  1. Navigate to the Checkout panel
  2. Scroll to Additional Fields
  3. Check "Enable Custom Checkout Form"
  4. Click "Edit Custom Checkout Form"

Create Your Form:

  1. Enter a form name (e.g., "Gift Message" or "Delivery Instructions")
  2. Click "Add Form Field" to create new fields
  3. For each field:
    • Add a title and description
    • Mark as required if necessary
    • Click "Done" to save the field

Customize Field Options:

  • Edit field content anytime
  • Remove fields using the trash can icon
  • Ensure at least one field exists
  • Check "Required" for mandatory fields

Form Display Location:

  • Physical products: Below shipping field
  • Digital products: Below email field
  • In-person sales: Form not displayed

Apple Pay Integration:

  • Form displays on order summary page
  • Non-required fields appear in collapsible menu
  • Customers can complete before proceeding to checkout

  • Gift messages
  • Delivery instructions
  • Referral surveys
  • Terms of Service agreement

Access Form Submissions:

  • Order details panel
  • Email notifications
  • Orders .csv export file

Test your form by running a test order to ensure it appears correctly to customers.
